Direct Deposit

Print the direct deposit form and complete all fields in the authorization form.

Attach a voided check with the authorization form.  The voided check should be from the account you would like the money to be deposited.

Return the completed authorization form with your voided check to your local Lakeshore Branch.

Don't have a checking account? Signup for a checking account through your local bank.
