Employees in good standing are eligible to receive bonuses based on hours worked.*

Holiday pay includes: New Year’s Day,

* Some client assignments are not eligible to receive these bonuses.
SUMMARY OF INSURANCE OFFERINGS
Below is basic information on the various insurance offerings made available to you. For more detailed information on any of these offerings you should refer to the plan documents which can be found in the Benefit Forms section of Lakeshore’s website. You may also call SRC directly at 888-772-9682 and provide them with group number 380730.
To qualify for benefits, you need to work for a minimum of six weeks with at least 20 hours per week.

Q: Who can I enroll?
A: In addition to you, dependent coverage is offered in the medical, hospital indemnity, dental, vision care and term life plans. Your eligible dependents are your lawful spouse and your unmarried children (who depend on you for support) from birth through age 18, through age 24 if a full-time student, or through any age if handicapped and unable to earn a living.

Q: When does coverage begin?
A: Your coverage is effective on the first day of the month after you enroll provided you are eligible. Employees become eligible when they have worked 6 consecutive weeks and averaged a minimum of 20 hours per week. You then have 30 days from the date you became eligible to enroll. If you do not enroll within 30 days, you are not able to enroll until the next annual open enrollment period. If you enroll dependents at a later date, their coverage will become effective after the written enrollment is approved and the premiums have been paid.

Q: When does coverage end?
A: Coverage, for all of your benefits under the Plan, will end if: 1) the required premiums are not paid, 2) you are no longer an eligible employee, 3) the policy terminates, 4) you enter an Armed Service on full-time active duty, or 5) you become eligible for another employer-sponsored group medical plan. Dependent coverage ends when yours does or when the dependent is no longer eligible.

Q: If I elect not to participate now and need to later, what are my enrollment rights?
A: You can enroll yourself and/or your eligible dependents at any time during your employment if you have experienced a qualifying life event (i.e.: marriage, divorce, adoption, birth of child). Enrollments are effective the 1st of the month following receipt of your enrollment form.

Q: Which doctors, dentists, and hospitals may I use?
A: You are free to use any licensed doctor or dentist, or any certified hospital. However, under the Medical Coverage you can save money by using an in-network provider.

Q: What is a beneficiary?
A: Your beneficiary is the person you name to receive the benefits of any term life or accidental death coverage you may have in the event of your death.

Q: Who can I name to be my beneficiary?
A: Anyone you choose. If you choose someone who is younger than 18 you will have to name an adult to be the custodian of the benefits until the minor reaches age 18.

Q: What is a deductible? A co-pay?
A: A deductible is the amount of money you must pay for eligible expenses before the Plan begins to pay benefits. You will stop paying toward the deductible when you reach the specified dollar amount. A co-pay is the amount you pay each time that you incur certain charges. You will continue to pay the co-pay even after you have reached your deductible.

Q: When will I receive my Summary Plan Description (SPD)?
A: SRC is no longer required to send out an SPD. If you’d like to receive additional plan information you should contact SRC directly.

Q: Will I receive plastic insurance cards?
A: If you enroll in medical or dental insurance you will receive a plastic ID card in the mail after your enrollment has been processed. Until you receive your plastic card you may use the temporary card found in your enrollment kit. The temporary card can also be printed from Lakeshore’s website under the Benefit Forms section.

When you enroll in an SRC medical plan, you also receive these additional benefits:
- Wellness benefits- Additional $100 per coverage year for preventative care.
- Employee Assistance Plan - (EAP), available 24/7 online or via toll-free number helps manage stress and balance work and life
- Informed Hotline – Toll free line connects you to a team of nurses experienced in providing information on a variety of health topics. Available 24 hours a day, 7 days a week.
- Aetna Pharmacy Management – In addition to prescription discounts, this service provides information to help control certain medical conditions.
- Alternative Health Care Programs
- Natural Alternatives – Discounts on alternative therapy services such as acupuncture therapy, chiropractic manipulation, massage therapy and nutritional counseling.
- Natural Products – Discounts on natural health products such as aromatherapy, biomagnetics, homeopathy, natural body care, yoga tools and more.
- Vitamin Advantage – Discounts on vitamins, nutritional and herbal supplements.
- Fitness Program – Enjoy discounted rates on memberships at participating health clubs contracted with GlobalFit™ as well as savings on home exercise equipment.
- Vision One® – Discounts on eyewear, contact lenses, LASIK eye surgery and eye care accessories. Participating optical centers include Sears Optical®, Lenscrafters®, Target Optical® and many Pearle Vision® locations.
HOSPITAL INDEMNITY COVERAGE
Daily in-hospital indemnity benefit pays for a non-occupational injury or sickness:
- Lump sum in-hospital benefit of $1,000
for one confinement per coverage year - Daily in-hospital benefit of $100 per day
of hospital confinement
DENTAL COVERAGE
- Covers most common services
- Use any dentist of your choice
- Receive additional savings by using one of the 76,000+ available PPO network dentists nationwide
- $500 coverage year maximum
- $50 annual deductible
- Coinsurance varies by service
- Waiting period required except for check-ups and cleanings
- Enrolled dependents receive the same coverage
- Network is not available in AL, AR, ID, HI, LA, MS or NM
VISION CARE
- $25 annual eye exam benefit
- Eyewear discount card offers savings:
- Up to 60% off eyewear purchases
- 10% off contact lenses and other optical items
- Eye exam benefit and discounts on eyewear purchases apply to each covered person
SHORT-TERM DISABILITY COVERAGE (STD)
- Pays up to 26 weeks
- 50% of base pay up to $125 per week
- 14 day elimination period; immediate benefit if hospitalized
- Not available in CA, HI, NJ, NY and RI, where employers are required to provide coverage
TERM LIFE WITH ACCIDENTAL DEATH BENEFIT (ADB)
- $20,000 of term life coverage for yourself
- $20,000 matching accidental death benefit
- If you sign up for term life for yourself, you can enroll your eligible dependents for:
- $2,500 in term life only for dependents over 6 months
- $500 for children 6 months of age or younger
- Your benefits are reduced by 50% at age 70. Dependent term life benefits end when they reach age 70
- Benefits will be paid to the beneficiary of your choice
