Lakeshore offers a comprehensive benefit package to our valued field employees; this coverage is provided by Aetna Affordable Health Choices and administered by Strategic Resources Company (SRC).

Employees become eligible to elect coverage when:

  • You have worked a minimum of 6 weeks with 20 hours or more in each of those weeks
  • You will still be on assignment on the first of the month following receipt of this notice
  • You did not decline coverage enrollment during a prior enrollment window

You will be eligible to elect coverage for 30 days from the date you have completed 6 weeks of work with a minimum of 20 hours per week. Enrollment will occur on the first of the month following receipt of your election form, if it’s received within your 30 day window of eligibility. If you do not elect coverage you will not be able to elect coverage until our next open enrollment period unless you experience a ‘life changing event’ (i.e.: marriage, divorce, loss of other coverage, etc.)

Medical Option 1:

$250 deductible, $10 office visit co-pay 5 visits, (plan pays 100% up to benefit maximum).

Medical Option 2:

  $250 deductible, $10 office visit co-pay 5 visits, (plan pays 100% up to benefit maximum)

Dental:

  $50 deductible, $500 annual maximum, no waiting period for check-ups and cleanings

Vision:

  Reimbursements of $25 for an exam once every 12 months

Short Term Disability:

  Pays up to $125 per week for up to 6 months, 14 day waiting period except for hospitalizations

Life:

  $20,000 Term Life coverage for employees, $20,000 matching Accidental Death benefit

Hospital Indemnity:

  Lump sum in-hospital benefit of $1,000 per coverage year, daily in-hospital benefit of $100 per day
